When a patient is physically or mentally incapable of signing the Assignment of Benefits form crews must also document "RUTS times 2", as described below, in order for the ambulance provider or supplier to make use of the 42 C.F.R. §424.36(b)(6) exception to the signature requirements:
Reason Unable To Sign - Reason the patient is physically or mentally incapable of signing.
and
Representative Unable/Unwilling To Sign - Whether there is no other authorized signer available or willing to sign on the patient's behalf.
